If you are interested in registering, please complete the following form, as accurately as possible. ONLY ONE REGISTRATION REQUIRED PER TEAM. 

 

We ask that you provide the contact information for the Coach and/or General Manager in the form below as they will be the main point of contact for this tournament. 

 

*It is important to note registration does not mean automatic eligibility to participate. Commissioners will review the registrant before acceptance of entry*

TEAM FEES: (Minimum 10, Maximum 16 players) 
$450.00 for 10 players 

$45.00 for each additional player over 10

 

You will be required to register and submit 

a payment of $45.00 to NSA at a later time. Please do not include this amount when sending your team fees to us.

1) TEAM NAME 

2) NUMBER OF PLAYERS: You must pay for 10 players minimum. If additional players are added to your roster, any transfers must include your team name for tracking purposes.
